Getting It Right: Elevating Outcomes and Reducing Risk with Your Incontinence Documentation and Assessment Process

This fast moving webcast will focus on the documentation issues related to the identification and treatment of incontinence in skilled nursing environments.  The proper documentation of incontinence is essential in the clinical record and in the MDS data base. 

Regulatory risk will be discussed as this issue can impact quality of life and rehospitalization rates.  Clinical staff and therapy staff must be aware of continence status and interventions to improve the status of continence. 

This session will review the definitions related to these issues – identify the regulatory risks – review interdisciplinary problem solving and communication to improve documentation and resident outcomes.  Discussion will also include the impact of toileting programs on overall outcomes and declines in rehospitalizations.  A very important clinical and administrative issue for all facilities.

Learning Objectives:

  • Review the issues related to documentation of incontinence and survey risk.
  • Identify the specific definitions in the RAI Manual and CAA references that impact documentation of incontinence.
  • Discuss regulatory risks and outcome documentation  issues if MDS data and clinical record content are inconsistent or inaccurate.
