Indiana is launching its first Program of All-inclusive Care for the Elderly (PACE) community, creating new options for senior care in the Indianapolis area. The program will be operated by Franciscan Senior Health & Wellness, sponsored by the Franciscan Alliance.
The PACE model provides team-based care in a community setting as an alternative to nursing home settings for those who qualify. Enrollees must live in a PACE service area, be 55 or older, have a certified need for skilled nursing care and be able to live safely at home. The community’s care teams will include physicians, registered nurses, dietitians, social workers, physical and occupational therapists and activities coordinators, among others.
The site brings the national PACE tally to 107 programs across 32 states.
