Cloud-based, mobile solution improves care transitions

When a patient is transferred from a hospital to an assisted living facility, skilled nursing facility or home-care setting, the transition of care can be complex and challenging.

A new cloud-based, Health Insurance Portability and Accountability Act-compliant mobile solution from Los Angeles, Calif.-based Zynx Health helps to create cross-continuum patient-centered care teams that can coordinate evidence-based care across all care settings. 

Among ZynxCarebook's features is a connectivity and messaging tool that allows multidisciplinary teams on the inpatient and post-acute sides to connect in real-time while transitioning patients from the hospital to other care settings.

Marin General Hospital in Greenbrae, Calif., reported that after launching this mobile care-management solution the staff saw a 68 percent improvement in care team communication, a 22 percent reduction in 30-day readmissions and a 0.5-day reduction in the average length of stay for patients aged 65 or more years.
