The competition was fierce among the program submissions for the 2015 Long-Term Living OPTIMA Award, but the external panel of judges ranked one program above the rest. Congratulations to Silvercrest Center for Nursing and Rehabilitation, a 320-bed skilled nursing facility in Briarwood, N.Y., for its cross-department program to reduce hospital readmisisons among chronically ill residents, many of whom are ventilator-dependent or are tracheostomized.

Stay tuned for news from our upcoming visit to Silvercrest and our upcoming indepth article on Silvercrest’s program—including how the program idea was conceived, implemented and managed, and how it has impacted residents’ quality of life.

Congrats, Silvercrest!
